Patient Access is our secure online service for patients over the age of 16yrs to order their repeat medication online. Due to the Covid Pandemic the appointment booking facility has been suspended. Prior to using Patient Access, you will need a personal logon. To request this, please complete and return the Patient Access Registration form below along with a close up photograph of yourself holding appropriate photo identification so that we can confirm your ID.

Please return your completed application form and photograph with ID by email to gram.[email protected]; alternatively you can send a printed copy of these documents to us by post.
On receipt we shall phone you to confirm that this is a genuine request and once verified we shall return your Patient Access login details by email or by post depending on how we received your request.
